Form

The Contact Forms screen displays all forms registered for collecting requester information during in-person support.

How to Access

Access: Service/ In-Person Service/ Form.

Forms are used to standardize the recording of data at the time of service, ensuring that the necessary information is filled in according to the defined operational flow.

In the listing, it is possible to view the Name of the form and its activation status, in addition to using the available actions for editing or deletion of the record.

The button New allows the creation of a new contact form, which can later be linked to the In-Person Service Types.

New Form

This screen allows the registration of a new Contact Form, used for the structured collection of applicants' information during the execution of in-person service.

The form created can later be linked to a Service Type, determining which data must be provided at the time of service.

To register:

  • Name: Field for defining the name of the contact form;

  • Contact Form Key: Allows selecting the previously registered key that will be used as the basis for the form's structure.

After selecting the key, the fields available for composing the form will be displayed, allowing the following configurations:

  • Active: Defines whether the field will be enabled for display on the form during service;

  • Required: Defines whether filling in the field will be required to complete the service;

  • Order: Allows defining the display sequence of the fields on the form;

  • Action: Allows deleting the field from the form structure.

After making the desired configurations, click Confirm to save the contact form.

Edit Form

This screen allows updating the settings of a Contact Form, used for the structured collection of applicants' information during the execution of in-person service.

Field editing:

  • Name: Allows editing the form name;

  • Contact Form Key: Allows selecting or changing the key used as the basis for the form structure;

  • Active: Defines whether the form will be available for use during service.

After selecting the key, the fields available for composing the form will be displayed, allowing the following configurations:

  • Active: Defines whether the field will be displayed on the form during service;

  • Required: Defines whether filling in the field will be mandatory to complete the service;

  • Order: Allows defining the display sequence of the fields on the form;

  • Action: Allows deleting the field from the form.

Only the ID cannot be edited, being shown only for reference, with a copy option.
